Band 5 Community Nurse up to £30,000 PA FTE

Chase Medical are recruiting for a Community Nurse to be based at a forward-thinking and ambitious service in Kirklees. The successful candidate will be an RGN, ideally with Community or Primary Care experience, and a qualification in Long Term Conditions.

The working hours for this post are flexible, and your personal commitments will be taken into consideration to improve your work/life balance. Evenings and Weekends are available so if you're looking for a new job to fit around your life, then this is the perfect opportunity for you!

The successful candidate will be working as part of the Integrated Care Team, visiting elderly and vulnerable patients in their own homes for Admission Avoidance and Long-Term Conditions Care.

The successful candidate will have drive and enthusiasm for delivering excellent patient care!
